Overview
In this full-time or part-time role, as the RMD, under supervision
of the CMO, you will have several administrative, supervisory,
business development, quality improvement and clinical
responsibilities. While providing day-to-day clinical care to
residents residing in facilities, you will practice and role-model
ethical and evidence-based decision-making to promote the vision
and mission of the company to other practitioners and staff.
Responsibilities
- In partnership with the CMO and other
company leaders, devises and executes on state level strategy for
business development and delivery of the Abode Care Partner model
of care across various health settings
- In partnership with the VP of Value Based Operations and the
CMO, devises and executes on value-based strategies for the state
to include utilization management and attribution strategies
- Direct patient care responsibilities between 75%-85% of time
spent in the clinical capacity
- Establishes an excellent rapport with frontline staff; regional
and executive-level leaders of client institutions
- Where needed, assists the CMO in the hiring of physicians in
full-time, part-time, or independent contractor roles
- Assists the CMO in the monitoring of the clinical performance
and suggests performance improvement of Abode Care Partner
physicians in the state using available dashboards
- Provides collaborating physician duties for on average of 5
nurse practitioners and physician assistants (based on state
regulatory parameters) to support them in their clinical roles, to
include audits on up to 10% of practitioner medical orders and/ or
documentation for each provider monthly. (Dependent upon state and
regional regulatory requirements)
- Provides high-quality complex and geriatric care to residents
and patients across settings in partnership with non-physician
practitioner and other interprofessional team members to an
identified panel of patients across various assisted living and
skilled nursing facilities
- Keeps high engagement with facility leadership teams, nurse
practitioners/physician assistants, patients, and families
- If needed, assumes medical directorships for skilled and/or
other senior living facilities
- Helps identify new growth opportunities with skilled facilities
and/or other senior living facilities in the market/region
- Participates in company frontline /backup call processes for
timely response to acute changes of condition to prevent avoidable
hospital transfers, or to meet other clinical needs
- Keeps a high focus on quality outcomes including but not
limited to care path compliance and productivity, prevention of
avoidable transfers and decrease in polypharmacy and costs of
care
- Attends required trainings, and meetings as mandated by the CMO
or other leaders to assure best care delivery by the company
- If needed, partners with the CMO to provide CMO-level services
to client organizations for the CMO Service Line
Qualifications
- Graduate from an accredited school of medicine and is a Doctor
of Medicine
- Two or more years of administrative leadership experience in
healthcare
- Clinical medical experience of seven years or more
preferred
- Board certification in Family Medicine, Internal Medicine, or
Geriatrics; qualification in Geriatrics preferred
- Practicing physician currently licensed under the provisions of
the State Board of Medicine
- Active unrestricted licensure to practice in state of
employment
- Current Drug Enforcement Administration registration
- Ability to communicate effectively with patients and other
healthcare support staff
- Ability to treat patients and their families with compassion
and understanding
- Ability to work for long periods of time with patients who need
special attention
- Ability to display patience with children and adult patients
who fear medical treatment
- Excellent computer and electronic record-keeping skills
- Excellent organizational and management skills
- Excellent decision-making, problem-solving and cognitive
abilities
- Travel up to 25%
