Day to Day Life at Custom

Provide care for our community of recovering patients, ushering them back to a life of independence.
Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
Educate our patients and their families on next steps in continuing care and improvement of day to day life including: nutrition, healthy living, and treatments.
Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.
Focus on the goal of improving the overall health of our community.
