Responsibilities The Supportive Care/Palliative Care Nurse
Practitioner (SPCNP) works in collaboration with HarmonyCares
Medical Group staff to provide comprehensive, quality palliative
care services to patients under the direction of the supervising
Regional CMO. The SPCNP will be responsible for providing patient
visits for palliative care patients, both new and established and
coordinating their plan of care with other designated staff,
especially the palliative care team. Patient visits will include
visits to patient’s homes, facilities and hospitals. In addition,
the SPCNP will engage in independent decision-making about a
patient’s healthcare concerns and goals of care and provide
palliative care services on an as needed basis. Collaborating
Providers will be available for consultation, collaboration, or
referral. Essential Duties and Responsibilities The main function
of the SPCNP will be to provide medical support and coordination to
palliative care patients and family members. This includes such
duties as: Provides medical expertise on pain management, symptom
management, and the evaluation of terminal prognosis to patient,
family members and staff: Ordering of appropriate therapies and
tests when needed Conducting in-person and/or telehealth visits
based on operational need Focus on patient experience and
identifying patient goals of care Ensure that the care being
provided is based on established palliative care guidelines and
standards Regularly contacts and collaborates with HarmonyCares
Medical Group (or other referring providers) regarding new patient
referrals and existing patients. Coordinate care with external PCPs
and other agencies (as needed) Interacts with attending physicians,
as necessary, regarding the care and treatment of patients and
works collaboratively with internal partners Performs patient
visits primarily in private residences, long-term care facilities,
assisted living facilities, hospital or other locations Provides
expertise and oversight of pharmacy formulary and medication
expenditures Collaborate with Chief Medical Officer and Executive
Director Independently commute to patients for visits and market

Knowledge, Skills, and Experience Advanced Practice Nurse with
appropriate active and unrestricted state licensure Valid NPI, DEA
and CTP Enrolled with Medicare or state Medicaid or eligible for
enrollment 3 years of home based or hospice care or relevant
experience Pay Transparency Individual compensation packages are
based on various factors unique to each candidate, including skill
