you will: - Complete speech, language, cognitive-communication, and
swallowing assessments - Develop individualized care plans tailored
to each patient’s goals - Provide therapy services based on
physician orders and best-practice guidelines - Coach patients and
caregivers on exercises, techniques, and home programs - Document
your visits clearly and promptly - Partner with nurses, therapists,
and physicians to ensure seamless care - Promote safety and help
reduce avoidable hospitalizations What You Bring to the Team -
Master’s degree in Speech-Language Pathology - Active SLP license
in Michigan (or Educational Limited License with required
supervision) - At least one year of SLP experience; home health or
hospice is a plus - Strong communication and organization skills -
A patient-first, compassionate approach to care - Reliable
