About the Role: As a Nurse LPN/RN at our nursing home, you will
play a critical role in delivering compassionate and high-quality
healthcare to elderly and chronically ill residents. Your primary
objective will be to assess, plan, implement, and evaluate nursing
care tailored to each resident's unique needs, ensuring their
comfort, safety, and well-being. You will collaborate closely with
interdisciplinary teams, including physicians, therapists, and
social workers, to coordinate comprehensive care plans. This role
demands vigilance in monitoring residents' health status,
administering medications, and responding promptly to any changes
or emergencies. Ultimately, your dedication will contribute
significantly to enhancing the quality of life for our residents
and supporting their families through empathetic communication and
care. Minimum Qualifications: Valid and current Licensed Practical
Nurse (LPN) or Registered Nurse (RN) license in the state of
employment. Completion of an accredited nursing program (LPN or
RN). Basic Life Support (BLS) certification. Demonstrated
experience in a long-term care or nursing home setting is preferred
but not mandatory. Strong understanding of nursing principles,
medication administration, and patient care standards. Preferred
Qualifications: Certification in Geriatric Nursing or Long-Term
Care Nursing. Experience with electronic health records (EHR)
systems. Advanced certifications such as ACLS (Advanced
Cardiovascular Life Support) or PALS (Pediatric Advanced Life
Support) if applicable. Proven ability to work effectively within
interdisciplinary teams. Additional training in wound care,
dementia care, or palliative care. Responsibilities: Conduct
thorough assessments of residents' physical and mental health upon
admission and regularly thereafter. Develop, implement, and update
individualized nursing care plans based on assessments and
physician directives. Administer medications and treatments
accurately while monitoring for side effects or adverse reactions.
Provide wound care, catheter care, and other specialized nursing
procedures as required. Monitor vital signs and report any
significant changes or concerns to the healthcare team promptly.
Assist residents with activities of daily living, including
mobility, hygiene, and nutrition support. Document all nursing
care, observations, and resident responses accurately and timely in
medical records. Educate residents and their families about health
conditions, treatments, and preventive care measures. Ensure
compliance with all regulatory standards, facility policies, and
infection control protocols. Participate actively in care team
meetings, quality improvement initiatives, and ongoing professional
development. Skills: The required nursing skills are applied daily
to assess resident health, administer medications safely, and
provide direct patient care with precision and empathy. Strong
communication skills enable effective collaboration with healthcare
team members and clear education for residents and families.
Critical thinking and problem-solving skills are essential for
identifying changes in resident conditions and responding
appropriately. Organizational skills ensure accurate documentation
and adherence to care plans and regulatory requirements. Preferred
skills, such as proficiency with electronic health records and
specialized certifications, enhance efficiency and the quality of
care delivered, supporting continuous improvement in resident
