Position Summary:
Provides health care services to Safe Harbor and Respite clients in accordance with professional standards and The Salvation Army Harbor Light Programs Policies and Procedures. To accomplish this, the Respite/Safe Harbor Care Coordinator acts as client advocate linking clients with both internal and external healthcare resources as well as daily monitoring of program participant's health and wellbeing.
Essential Responsibilities:


Manage all inquiries with regards to information, referral, and/or possible admission.
Conduct pre-admission screenings of potential clients for Safe Harbor and Respite clients, schedule admissions according to Harbor Light admission criteria.
Conduct client admissions/discharges for Safe Harbor and Respite programs, including completion of all required medical documents.
Perform hourly checks on Safe Harbor and Respite clients.
Perform instant urinalysis drug screenings.
Provide general health care of clients which includes but is not necessarily limited to:

Administering and recording medications as prescribed by attending physicians.
Perform vital sign checks and document results.


Facilitate health education groups as required by Safe Harbor and Respite Program Director and or Medical Director clients.
Take appropriate action as directed by attending physician to ensure client's medical stability and in cases of emergency facilitate assistance per Harbor Light protocol.
Assist Medical Director or designate in the provision of medical services.
Attend all required staff meetings; training sessions, seminars, and/or workshops for staff development
Coordinate client medication needs with pharmacies.
Maintain records, charts, logs and all other pertinent data for the medical unit on a daily basis.
Educate, coach and assist resident in establishing appointments and navigating managed care system.
Assist residents with obtaining and engaging primary care physician, managed care liaison, and social services.
Research and maintain working knowledge of current trends and best practices in respite care.
Participate in weekly case review meetings.
Conduct necessary assessments: VI-SPDAT, AZ Self-Sufficiency Matrix, Discharge, Barrier assessment.
Consult with internal and external service providers in cases where more in-depth counseling may be needed regarding mental health, substance abuse, physical abuse, etc.
Be available to give support, assistance, and supervision to residents when necessary.
Assist residents in establishing and maintaining appropriate conduct while in residence; resolve residents' behavioral issues in a timely manner.
Drive clients to outside appointments.
Perform other duties as assigned.


Additional Responsibilities:

Qualifications:
Education/Experience:

Successful completion of an accredited EMT, Paramedic, and or LPN program, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
Certified CPR Trainer (preferred)
Experience in substance abuse (preferred)


Skills, Knowledge & Abilities:

Possess working knowledge of Occupation, Safety, and Health Administration standards and Universal Precautions


Computer Skills:

Proficient in Microsoft Office 365
Working knowledge of TEAMS and SharePoint
Experience with Electronic Medical Records preferred
Willingness to learn new software as needed


Certificates and Licenses:

Complete Safe From Harm training, and keep current as needed
Must have and maintain a current valid chauffeur's license and pass the Salvation Army Motor Vehicle Record check (MVR); or obtain the chauffeur's endorsement within 30 days.
Must possess one of the following Michigan licenses:

Registered Nurse, Practical Nurse
Emergency Medical Technician
Paramedic
